The instance group required the named port to be mapped to a port number. I tried this with the gcloud compute instance-groups set named-ports but this didn't seem to take effect until I also did it through the console.

VM instances were not detected even though they had green checks and the Firewall rule was implemented to allow healthcheck IP ranges to access tagged instances as http to accept ingress on port 80.
